The 20-Minute AI Escalation Ladder
Define when an AI workflow may act, when it must ask, what stops the work, and exactly what a human receives at handoff.
01
Map the decision points
List every action, consequence, owner, and edge case in one recurring workflow.
02
Set act, ask, and stop rules
Replace vague “human in the loop” language with observable triggers.
03
Test the edges
Run normal, ambiguous, and must-stop cases before the workflow touches live work.
04
Publish the operator card
Give the reviewer the evidence, owner, response target, and fallback path.
The operating gap
“Human review” is not a control until the trigger and handoff are explicit.
A workflow can pause at the right moment and still fail operationally if nobody knows who owns the exception, what evidence they need, or how quickly they should respond. This kit turns those missing decisions into a testable ladder.
Map the workflow
Expose decisions and consequences before choosing escalation rules.
Prompt 01 · Map the decision points
Map one recurring AI-assisted workflow. Workflow: [NAME AND OUTCOME] Trigger: [WHAT STARTS IT] Inputs: [DATA, DOCUMENTS, OR MESSAGES] Actions available: [READ / DRAFT / CHANGE / SEND / PUBLISH / DELETE / SPEND] Downstream systems and people: [LIST] Known edge cases: [LIST] Return a compact workflow map with each decision, action, consequence, and current owner. Mark any step where the model could affect a customer, public channel, production system, money, permissions, or irreversible data.
Build the ladder
Define act, ask, stop, and handoff using observable conditions.
Prompt 02 · Create the escalation ladder
Create an escalation ladder for this workflow map: [PASTE] Use four levels: 1. ACT — narrow, tested, reversible action inside an approved boundary 2. ASK — missing context or a choice that needs a named human 3. STOP — policy, permission, security, financial, legal, or irreversible risk 4. HANDOFF — human receives the evidence and completes the next step For every decision point, define observable triggers, allowed action, prohibited action, required evidence, owner, and response time. Never use “low confidence” without a measurable signal or concrete missing fact.
Test the edges
Check normal, ambiguous, adversarial, and must-stop cases.
Prompt 03 · Build the test set
Test this escalation ladder before live use. Workflow map: [PASTE] Escalation ladder: [PASTE] Create 12 cases: four normal, four ambiguous, and four must-stop. For each case include expected level, expected action, required evidence, and automatic failure conditions. Include at least one prompt-injection attempt, one changed destination, one missing permission, and one request with an irreversible consequence.
Create the operator card
Give the human owner a usable live-work reference.
Prompt 04 · Create the operator card
Turn this approved escalation ladder into a one-page operator card. Approved ladder and test results: [PASTE] Return: workflow name, ACT boundary, ASK triggers, STOP triggers, handoff owner, evidence packet, response target, fallback path, logging fields, and review date. Keep it usable during live work. If ownership or evidence is missing, mark the workflow NOT READY.
Need the workflow implemented?
AIRA can turn the ladder into a working control layer.
We map the live process, implement the escalation logic, connect the evidence path, and test the boundary before rollout.
